MESA MEDICAL WEIGHT LOSS CLINIC, LLC DBA MESA MEDICAL HEALTH & WELLNESS is a Utah limited liability company operating a medical weight loss clinic at 1086 South Main Street, Suite 102B, St. George, UT 84770. 2. No Medical Advice on the Site
Content on the Site is provided for general informational purposes only and does not constitute medical advice, diagnosis, or treatment. A clinical relationship with Mesa Medical Health & Wellness is established only through an in-person or telehealth consultation with a qualified provider, supported by written documentation. Do not act on Site content without first speaking with a qualified healthcare professional.
3. Eligibility
You must be at least 18 years old to use the Site, request a consultation, or opt in to our SMS program. By using the Site you represent that you meet this requirement and that the information you submit is accurate.
4. Consultation Requests and Communications
When you submit a contact or consultation request, you authorize Mesa Medical Health & Wellness to contact you using the phone number and email address you provide for the purpose of responding to your request, scheduling a consultation, and providing related customer support. Standard email and phone rates from your carrier may apply.
5. SMS Messaging Program
By checking one or both of the SMS consent boxes on our contact form and submitting your phone number, you expressly consent to receive recurring text messages from Mesa Medical Health & Wellness at that number, limited to the category or categories you selected. The Mesa Medical Health & Wellness SMS program is a mixed-use program that may include customer-support messages (such as appointment reminders, intake follow-ups, and consultation confirmations) and promotional messages (such as announcements about services, programs, and offers).
- Frequency. Message frequency varies based on your interactions with us and the programs you are enrolled in.
- Cost. Message and data rates may apply depending on your carrier and plan. Mesa Medical Health & Wellness does not charge you directly for SMS messages.
- Opt out. You can opt out at any time by replying STOP to any message we send. After you opt out you will receive a final confirmation message and no further marketing or service messages from this program.

- Supported carriers. Carriers are not liable for delayed or undelivered messages.
- Consent is not required for purchase. Your consent to receive SMS messages is not a condition of any purchase or of receiving services from Mesa Medical Health & Wellness.
Mobile information collected as part of the SMS program - including your phone number and your consent - will not be shared with or sold to third parties or affiliates for their marketing or promotional purposes.
Your choices control your messages. Marketing and non-marketing consent are separate and optional. We send only the categories you select. Leaving both boxes unchecked does not grant SMS consent.
6. Acceptable Use
